The enVista Envy™ Toric for Precision Treatment of Astigmatism
In a recent conversation, Dr. Mitch Shultz, a refractive surgeon in private practice in Northridge, California, discussed why it is so important to correct as much astigmatism as possible during cataract surgery. Dr. Shultz also explained why he believes that the new enVista Envy Toric IOL with its Low-Cyl technology provides the surgeon with the most precise treatment of astigmatism of any full range of vision toric platform on the market, uniquely treating half-step cylinders between 1.25 D and 3 D at the IOL plane.1
Residual, Uncorrected Astigmatism Has Consequences
A significant number of cataract patients with astigmatism go uncorrected.2 Uncorrected astigmatism can result in decreased quality of vision due to lack of vision clarity, visual disturbances, and dependence on eyeglasses.2 Studies have shown that patients with >0.5 D of residual astigmatism who received a full range of vision IOL lens see a significant reduction in their quality of distance vision.3,4 Even a small amount of astigmatism (as little as 0.5 D) has the potential to affect the patient’s functional and low-contrast visual acuity and the ability to perform computer tasks comfortably.5,6
This is important because patients have high expectations for a premium IOL, and residual astigmatism after cataract surgery can leave patients dissatisfied and desiring corrective treatment.7 Dr. Shultz noted, “Patient satisfaction is the most important factor that drives our profession. Patients today are looking to social media as a source of validation, and unhappy patients are the most vocal and can really do damage to your reputation. We all want to limit that risk. Happy patients may not sing your praises online, but they will refer friends and family to you. In contrast, unhappy patients can crush a practice.”
Low-Cyl Technology Provides Greater Precision
For the frequent occasions when patients have presbyopia and astigmatism, the enVista Envy Toric IOL allows the surgeon to correct both at the same time.1,8
With its Low-Cyl technology, the enVista Envy Toric expands premium options for low astigmatism treatment as the only IOL platform in the US that corrects less than 1.0 D astigmatism at the corneal plane compared with available multifocal toric platforms.9 This is beneficial as about three out of four cataract patients have less than 1.25 D of corneal astigmatism.10

The enVista Envy Toric IOL has the widest range of astigmatic treatment in terms of cylinder correction.1,11,12 It offers the lowest cylinder treatment available in the US — 1.25 D at the IOL plane, <1.0 D at the corneal plane, and half-steps in cylinders between 1.25 D and 3 D (which is suitable for approximately 23% of cataract patients).1,10-12 These features provide options for the treatment of astigmatism for more patients and with more precision.1
Finally, the enVista IOL is built on a glistening-free platform* and provides lasting clarity and full range of vision. It is exciting to be able to provide patients with full range of vision, an enviable dysphotopsia profile, and precision astigmatism management.
*Glistenings are observed by ophthalmologists at the slit lamp as backscatter. There has been no established correlation between these backscatter observations and what patients observe.
